9.           7:15 p.m.  Public hearing for a proposed amendment to the Comprehensive Land Use Plan, adopted in 2008, as requested by Arlington Cemetery, 4001 South 27th Street, to change the classification for approximately five acres of vacant/open land at the northwest corner of Arlington Cemetery from ‘Mixed Residential’ to ‘Community Facilities’ for a potential location of a worship site for the Church of Jesus Christ of Latter-Day Saints with the balance of the 100 acre Arlington Cemetery parcel designation of ‘Community Facilities’ remaining unchanged and no rezoning necessary. 10.           7:20 p.m.  Public hearing for approval of a Special Use Amendment, as requested by Dr. Dawg LLC, to construct an outside uncovered fenced patio on the south side of the building located at 7700 West Layton Avenue and to expand their designated ‘alcohol premise area’ to also include this patio, Tax Key #605-9944-006

11.            Plan Commission Meeting held November 13, 2012  (Kastner) (action taken on Items 4, 5, 7 & 8 at 11/20/12 Council meeting)


Item #6  Approve a Special Use Amendment/Site & Landscaping Review for 7700 West Layton Avenue to have an outside uncovered fenced patio on the south side of the building at this location, and a Plan Commission sign waiver for a pylon sign instead of a monument style sign, due to line-of-sight issues for vehicles leaving the west driveway and at a 13’ height from grade rather than the 19’ height that was initially presented and without the “hot dog” and “fork prongs” on top of the sign, subject to Plan Commission and staff comments, Tax Key #605-9944-006.  [[Action on this item will occur after the public hearing scheduled for 7:20 p.m. which pertains to the request to expand the designated ‘alcohol premise area’ to also include this proposed patio.]]


12.            Finance & Human Resources Committee Meeting held November 28, 2012  (Saryan)


Item #3   Approve renewal of election hardware maintenance agreement with Command Central for 22 Optech Eagle voting machines


Item #4   Approve renewing insurance package with the League of Wisconsin Municipalities Mutual Insurance for 2013 through R&R Insurance:  property with Chubb, worker’s compensation with United Heartland, crime and public officials with CNA Surety, and storage tanks with Nautilus; and authorize the Mayor to sign paperwork for the self-insured retention, and for the rejection of terrorism insurance (Berkley and Chubb)


Item #5   Approve contract between the State of Wisconsin Department of Health and Family Services and the Greenfield Health Department related to 2013 Prevention grant funding (9/1/12 – 8/30/13)


Item #6   Approve contract between the State of Wisconsin Department of Health and Family Services and the Greenfield Health Department related to 2013 Consolidated Contract grant funding (1/1/13 – 12/31/13)


Item #7   Approve filling a Part-Time Public Health Nurse position due to an anticipated retirement


Item #8   Approve a job description for the position of Administrative Assistant – Health Department


Item #9   Approve creating and filling an Administrative Assistant position in the Health Department due to an anticipated retirement


Item #10   Approve amending the contract with Independent Inspections, Ltd. to continue providing fill-in building inspection services on an on-call/as-needed basis through 12/31/13


Item #11   Approve agreement with Key Benefit Concepts for an updated OPEB (Other Post Employment Benefits) valuation as of 1/1/13


Item #12   Approve amending Resolution No. 3412 to set the salaries and benefits of non-represented and command staff employees of the City of Greenfield for 2012


Item #13   Approve schedules of disbursements in the amounts of $866,780.43 and $104,620.81
